Inland Empire Health Plan

In partnership with Partners for Better Health, Riverside County Public Health, San Bernardino Public Health and the Georgia Health Policy Center, Impact4Health was responsible for facilitating a community centered collective impact initiative to address the social determinants of health linked to diabetes and to craft a five year strategy for improving access to prediabetes prevention programs to low income and vulnerable populations.

Impact4Health provided a train the trainer program for the Inclusive Leader 360 for its senior executive team to support its long term strategy to create a culture of inclusion that advances health equity in the County.

Impact4Health is responsible for developing and facilitating a workshop series for its membership called “Diversity, Inclusion and Cultural Competency: Strategies for Advancing Health Equity” that was delivered live and and as webinar.

Sutter Health (Inclusive Leader Workshops)

Between 2016 - 2019 Impact4Health designed and facilitated the Inclusive Leader Workshop attended by over 700 managers and above. This was a collaborative effort with the internal Diversity and Inclusion leadership team to support awareness of the role unconscious bias plays in healthcare and to engage leaders in supporting a culture of Inclusion.

Sutter Health (Inclusion Council Training)

Impact4Health designed and provided an orientation and planning session for newly formed Inclusion Councils throughout the systems 24 Hospitals and 33 Urgent Care Centers.
