The Inclusion SCORECARD for Population Healthâ„¢ is a customized on-line dashboard of best practices designed for healthcare leaders to address health care inequities as part of a comprehensive Population Health Management Strategy.  The Scorecard contains over 70 best practices that address four distinct areas of activity: tracking key metrics about the population served and the overall diversity of staff at all levels, building a culture of inclusion throughout the health system, creating greater accountability among leaders to address health disparities and developing higher engagement with diverse community stakeholders.  While many of these activities may be conducted in isolation, the Scorecard inspires greater coordinated and systematic approaches to address high or rising risk patients across the entire organization and has the option to include key community organizations that are also involved in healthcare coordination, upstream social determinants of health or health education.
